The Health & Social Care Information Centre (HSCIC) has the authority to extract information from our records to support treatment and care.
The Personal Confidential Data (PCD) to be shared will include postcode, NHS no, date of birth and gender, as well as medical information such as diagnoses, referrals, family history, vaccinations, results, and prescriptions but not free text information.
Patients do have a choice to opt out. If you would like to do so then please complete our summary care record form.
